| Things have been getting lost all the time. This is only getting out because it's flavour of the month and insider contacts now know to tip off the media whenever it happens. |

I agree but it IS happening and that's what is important. Systems and controls should be in place before this happens, not after. Let's hope insiders do keep tipping off the media for the sake of the people it affects. |

| I think as long as human beings are involved f***-ups will be happening. But absolutely, tell the media because they are the only ones who properly hold individuals or authorities to account. |

| I think the people responsible should be named and shamed. Then they should be sacked - lose there pension benefit entitlement - and not be allowed to work in any government organisation again. |
